<image:title>Neuropathic Pain</image:title>
<image:caption>Acute pain is protective in nature. Upon sustaining an injury, our pain reflexes kick in the form of acute pain to reduce further harm. However, when our nerves, which are responsible for sending signals to our muscles, are injured, these signals may go haywire. A case of damaged nerves can be caused by chronic illnesses, physical accidents or other conditions and illnesses without visible injuries. The medical description for pain that arises from nerve damage is neuropathic pain. While pain signals are all transmitted by the nerves in our body, nerve injury can produce different signs and symptoms. For example, neuropathic pain can present itself as electric shocks, extreme cold or burning sensations, be accompanied by skin changes or even osteoporosis. The affected limb may also become extremely sensitive to touch.</image:caption>

<image:title>Neuropathic Pain</image:title>
<image:caption>While this category of pain is usually identified from such observed symptoms carried by the patient, a further clinical examination can prove to be useful. This is because the pain may not only arise from nerve damage. A professional will evaluate any possible chronic protective postures or movements, or even the lack thereof – adding another dimension of the patient’s muscular pain. Following the examination, here are a few standard treatment options in Singapore that are meant to be sought in complement with one another and not in isolation. Pain medication The intake of regular painkillers (paracetamol, anti-inflammatories, opioids) are not effective in treating neuropathic pain. While these painkillers may provide temporary relief, an over-reliance is likely to lead to an increase in dosage but with a lowered level of effectiveness. Should the patient require specific neuropathic pain medication, the doctor will discuss the prescription in detail. Physical therapy Physiotherapy may be beneficial in strengthening and mobilising weakened limbs. Other avenues of physical therapy include desensitisation or electrical stimulation therapy. The use of such nerve physiotherapy helps to bring nerve pain relief to the patient. Pain intervention Pain intervention involves giving the affected nerves a rest. This is typically done by injecting the patient with local anaesthetic and other medications or giving compressed nerves the space to mobilise. These may be performed with the guidance of an X-ray or ultrasound scan. In cases where surgery may be beneficial, an appropriate surgical referral will be made for the patient.</image:caption>

<image:title>Musculoskeletal Pain & Muscle Tear Treatment</image:title>
<image:caption>Musculoskeletal pain and discomfort are prevalent in any rural or urban society, Singapore included. The sources of such pain may be obvious, such as the result of precipitating incidents. However, the cause of the pain can also be either insidious and arise from the overuse of injured muscles, or seemingly innocuous and stem from sedentary lifestyles. Precipitating incidents include accidents, falls or forms of exercise. These events are typically traumatic and may force the body into positions it is usually not used to. Furthermore, these events may stress your muscles and tendons, causing strains and tears that appear as pain. For instance, a car accident can lead to pain in the knee or ribs, where you feel tenderness in these affected areas and perhaps breathlessness if there is a case of an intercostal muscle strain. Apart from the above-mentioned incidents, long term musculoskeletal pain may present itself gradually. For instance, an office worker can start feeling a strain in their wrists after two to three months of constant typing, or perhaps even lower back and neck pain after many months and years of prolonged sitting in front of the computer screen.</image:caption>
</image:image>
<image:image>
<image:loc>https://images.squarespace-cdn.com/content/v1/5ff49699d930bf2c1f8fb053/1627191909994-ZMHBDDLM4B9HU0MQSQVS/musculoskeletal-knee-pain-singapore.jpeg</image:loc>
<image:title>Musculoskeletal Pain & Muscle Tear Treatment</image:title>
<image:caption>This type of pain usually arises because of muscular imbalance or weaker postural muscles forced to work overtime. However, lower back pain may also be caused by prolapsed discs; a constant poor sitting posture can build unnecessary pressure at susceptible discs and cause them to tear. Additionally, people who lead inactive lifestyles tend to suffer from musculoskeletal aches and pains. These may arise from the constant joint stress – primarily from getting injured while the body is in a state of deconditioning (a process whereby the body experiences a physical or psychological decline in function) or from weight gain. What can you do to treat musculoskeletal pain? This category of pain is commonplace, and many see the treatment as simple as purchasing off-the-counter medication, ointments and getting adequate rest. However, persistent aches, if untreated, may result in chronic pain. While a patient suffering chronic musculoskeletal pain may have recovered from the inciting injury, the pain can still persist in the area. This is due to the pain signals getting fired up and amplified.</image:caption>

<image:title>Musculoskeletal Pain & Muscle Tear Treatment</image:title>
<image:caption>From the time the ailment starts up till seeking medical treatment, other external sources may also exacerbate the site of pain, resulting in more than one irritant source. Hence, treatment starts with careful and professional assessment, in order to delineate the various possible sources of pain. While medication has been proven helpful, the main intent is to reduce the pain sensation so that the patient may commence physiotherapy and return to normality. Typically, opioids are avoided in the prescription. Lastly, specific pain interventions may be helpful to complement medication and physiotherapy. It’s best to discuss the benefits and side effects with your pain physician to aid decision-making in your treatment. Here at The Anaesthesia And Pain Practice, we take pride in offering highly safe and comfortable treatments in Singapore that are tailored to your medical condition. Entrust us with your rehabilitation journey; our team of skilful and certified anaesthesiologists strive for expeditious yet pleasant recoveries for all our patients.</image:caption>

<image:caption>While discomfort in the lower back and neck areas may begin as innocuous aches and pains, if the discomfort persists for more than three months, you might have contracted a case of chronic lower back and neck pain. This chronic condition may have been initiated by events such as falls or daily overuse of the muscles – leading to prolonged strain, imbalance, and even possible degeneration of your muscles. As an ailment that commonly affects both men and women, the young and old, chronic lower back and neck pain can be considered a worldwide epidemic. With that being said, our fast-paced society here in Singapore is also not spared. While the contributing factors of the condition may differ for developing and developed countries, some of the more common factors include:</image:caption>

If you have been using your neck, shoulder or core muscles often, the presence of muscular imbalance will lead to aching pain. Unfortunately, if the condition turns chronic, a nagging pain is often observed. · Chronic strain Singapore’s work ethic is said to be a hardworking and honest one that has led to notable economic success. However, a culture of long working hours brings prolonged standing or crouching over a computer screen, inevitably leading to back and neck pains. Furthermore, with the looming COVID-19 pandemic that encourages the work-from-home arrangement, the ergonomics of this new working culture has contributed to the rise of these aches and pains. · Accidents Sudden movements, strains and falls will also impact the multitudinous ligaments of the back and neck. Apart from that, slipped discs and degenerative joints can also contribute to chronic lower back and neck pain.</image:caption>

<image:caption>Such pain is usually present as nagging, aching or sharp discomforts and may spread to the arms or legs if the nerves are affected. Hence, pinpointing one culprit as the source of pain may oversimplify things. If you are experiencing persistent pain, it is best to seek pain consultation and undergo history taking with a thorough physical examination. This would be crucial in helping the doctor arrive at an accurate diagnosis and comprehensive treatment plan. In the process, the specialist might suggest imaging scans or other examinations (such as nerve conduction tests) to complement, confirm or exclude any other possible diagnoses.</image:caption>

<image:caption>While hectic schedules are commonplace in Singapore, Singaporeans generally still try to maintain a healthy and active lifestyle. You would frequently catch brisk walkers, joggers and cyclists working out during the cooler parts of the day. Moreover, when the safety measures permit, gyms are always packed with avid fitness enthusiasts. Unfortunately, with increased physical activity, our bodies are exposed to higher risks of wear and tear, which could lead to injury. Even those who experience a sudden increase in activity after previously adopting sedentary lifestyles are not spared.</image:caption>

<image:title>Sports Injury</image:title>
<image:caption>How do these common sports injuries arise? Typically, these injuries are found at the body parts that perform the most movement or are exposed to the greatest amount of stress daily. A non-exhaustive list of common sports injuries includes neck strains that extend down to the shoulders, shoulder injuries, golfer's or tennis elbow, lower back and hip pain, knee and ankle strains, as well as aches and plantar fasciitis. Aches and discomfort can originate from the joints and the surrounding soft tissue or muscles. After prolonged physical activity, these joints may get worn out, and soft tissue such as ligaments may get strained or torn. While muscle tears are uncommon, these tears are usually present in the smaller muscles and can affect muscle movement in areas like the shoulder and hip rotator groups. Tendons, which are fibrous connective tissues that link your muscles to bones, are also commonly strained. A common tendon injury is the Achilles tendonitis condition. Stretching from your calf muscles to the heel bone, the Achilles tendon aids in regular walking and running. As such, this tendon is highly prone to being overused and inflamed, leading to Achilles tendonitis.</image:caption>

<image:title>Sports Injury</image:title>
<image:caption>How are such injuries typically diagnosed? When the area of pain is discrete and congruent with the injury site, clinical assessment for the patient would be straightforward. However, at times, pain in the neck, hip and back may encompass a huge area and cause the diagnosis to be tricky. For instance, more than one muscle or tendon may be involved in shoulder and hip rotator injuries. Additionally, plantar fasciitis, an inflammation that causes pain in the heel of your foot, might be a seemingly simple injury but can also lead to tiny nerve compression. If there has been persistent pain and suspicion of muscle tear, it’s best to see a professional immediately. Your doctor is likely to perform tests such as an ultrasound scan or MRI imaging to confirm the diagnosis and develop solutions for injury management. Apart from prescribing medication for your condition, treatment typically includes physiotherapy and imaging-guided injections. Specifically, ultrasound-guided injections are highly accurate as they allow the doctor to control the needle placement in real-time, avoiding potentially dangerous areas such as blood vessels.</image:caption>

<image:title>Headaches & Facial Pain Treatment</image:title>
<image:caption>Headaches are universal and affect people across all age groups. Although discomfort in the head is generally identified as headaches, there are more than 150 types of headaches – the more common ones are migraines and tension headaches. The cause of these head pains can vary. Apart from headaches with medical implications, such as those caused by toothaches, blood vessel and nerve abnormalities or even brain conditions; the origin of a headache may be as innocuous as initiated by stress, arise from injured nerves or neck pain, or as a result of other minor causes that are difficult to diagnose. Moreover, any head or facial pain also complicates the condition and is loosely interpreted as a headache.</image:caption>

<image:title>Headaches & Facial Pain Treatment</image:title>
<image:caption>Secondary headaches: underlying medical conditions Typically, there are two types of headaches, primary and secondary headaches. Secondary headache is a type of head discomfort that arises from an underlying medical condition. Sources of secondary headaches include toothaches, sinus congestion, or bleeding in the brain. Patients with secondary headaches are managed by their respective medical or dental specialists. However, the next type of headache should be considered when the ache or pain persists long after all bodily causes are ruled out. Primary headaches: inflammations at the neck and head On the other hand, primary headaches are caused by inflammations typically in and around the head and neck – the pain-sensitive parts of your body. Sources of primary headaches, which can also cause facial pain, include the overactivity of blood vessels, nerves and muscles. Primary headaches are usually labelled as migraine, tension, hypnic or cluster headaches. For primary headaches, it’s crucial to undergo a careful assessment to understand the pattern of headache attacks, the location on the head or neck, and the triggers or aggravating factors. Occasionally, a medical examination is needed to examine the headache thoroughly.</image:caption>

<image:title>Headaches & Facial Pain Treatment</image:title>
<image:caption>Treatment options for primary headaches Treatment for primary headaches is sought through medical options. Fortunately, most headaches are treated with minimally invasive techniques – rest and prescriptive or over-the-counter medication. Physical therapy may also be helpful in headaches related to musculoskeletal neck pain. To prevent an overreliance or overuse of medication, seek a specialist opinion for a safe treatment option. Additionally, if your headaches have been persistent, become more severe or frequent, let your doctor know so they can perform a thorough diagnosis and craft the preferred treatment for you. They might request further imaging tests to rule out any possible medical conditions. Here at The Anaesthesia and Pain Practice in Singapore, you will be treated with a comprehensive assessment, in the care of our highly qualified pain physican. We follow a thorough process: headache assessment, physical examination, advanced imaging and treatment option proposals suited to your condition.</image:caption>

<image:title>Elbow Pain</image:title>
<image:caption>Surrounded by strong muscles, namely the biceps, triceps and forearm muscles, our elbows are the silent workers of our body. Additionally, the elbow seems to function more simply – alternating between bending and straightening. When our wrists and shoulders ache, we are acutely aware. But, when pain is experienced at the elbow, a non-weight bearing joint that rarely needs joint replacement, many tend to overlook the discomfort. However, just like any other part of our bodies, we should not ignore persistent or prolonged pain at the elbow. In fact, the elbow is just as susceptible to injury as other bodily parts. This is because of the frequent use of the elbow joint and the stress (placed around the joint and ligaments) caused by the surrounding strong muscles. While chronic strain may be the primary source of pain, sudden movements against resistance, such as breaking a fall, may also be culprits. Elbow injury or pain occurs on either side of the joint, also commonly known as Golfer's Elbow or Tennis Elbow, depending on the location. While pain accompanied by occasional swelling may also arise from the front or the back of the elbow, it is more common to experience discomfort on the sides.</image:caption>

<image:title>Elbow Pain</image:title>
<image:caption>Tennis Elbow (lateral epicondylitis) Tennis Elbow is pain experienced when the forearm muscles on the outer side of the elbow are strained, resulting in an inflammation of these muscles. It is named as such because the injury arises from a tennis back-stroke motion; this condition occurs when there is an overuse of forearm muscles. Hence, Tennis Elbow pain is usually experienced by plumbers, cooks and carpenters. The risk of contracting Tennis Elbow is also higher if you are between 30-50 years of age or play racquet sports but do not practise the proper stroke techniques. Symptoms include: Wrist pain (during or after wrist use) Weak grip (difficult to lift or grasp objects) Persistent ache (in elbow area) Pain or tenderness (on the outer side of forearm to the wrist)</image:caption>

<image:title>Elbow Pain</image:title>
<image:caption>Golfer’s Elbow (medial epicondylitis) On the other hand, Golfer’s Elbow is the pain experienced with an inflammation of the forearm muscles on the inside of the elbow. It is named as such because this condition typically arises with an overuse of the forearm’s common flexor muscles, common while playing golf. However, Golfer’s Elbow is also prevalent among patients who regularly use their upper limbs in other sports or long hours of work, or even sustained a traumatic fall which affected these muscles. Symptoms include: Pain when twisting the wrist or making a fist (inside of elbow area) Pain when lifting heavy objects with palm facing up Stiffness in elbow Reduce strength in wrist or hands</image:caption>
